Job Summary

The Social Worker addresses member needs that require the services of multiple professionals, agencies, healthcare facilities and human service programs. The goal is to ensure coordination so that members with brain injuries receive the care and opportunities to which they are entitled. This position also is responsible for providing short-term counseling in order to make certain of successful completion of the member's goals. Social Worker may also assist members in the work activities of the program, as well as, in the community. In addition to the "work ordered" units, there are also educational groups focused on adjustment, substance abuse, hygiene and other relevant topics. Services are conducted to provide an environment to increase the member's independent living, prevocational, and vocational potential

Brief Summary:

The Social Worker coordinates with multiple agencies, healthcare facilities and human service programs to ensure Members receive the care, services and opportunities to improve their independence and overall quality of life. The Social Worker also leads educational life skills groups focused on self-advocacy, safety and prevocational skills as well as groups on adjustment to brain injury.


Overall Responsibilities:

  • Manages relationships with and delivers presentations to referral and funding sources
  • Conducts intakes and tours for new referrals as well as gathers all medical, insurance and ID documentation
  • Participates in staff and case conferencing meetings
  • Works with caretakers to ensure Members’ health, socialization, recreational and emotional needs are met
  • Coordinates and facilitates family meetings to discuss Members’ progress, goals, challenges and incidents
  • Writes progress notes as well as reports and letters to funding sources and relevant agencies
  • Regularly updates emergency contacts, healthcare providers and medication information
  • Reviews Member Manual, Photo/Video Approval and Release of Information Forms with Members
  • Manages orientation process for new Members
  • Assists Members with securing funding, transportation, housing, SSI/SSDI, Medicaid, Medicare and commercial insurance.
  • Provides one to one short-term counseling and referrals
  • Generates monthly attendance reports for the billing department
  • Updates and shares community resources in the Clubhouse as well as through email and social media
  • Plans and coordinates workshops for families and caregivers on a variety of topics including: maximizing entitlements, guardianship, housing, special needs trusts, Medicaid/Medicare and SSI/SSDI


Formal Education and Job-Related Experience
  • Bachelor's degree required 
  • Master's degree preferred
  • Experience working with individuals with disabilities in a non-profit organization preferred
  • Bilingual a plus

American Heart Association BLS for Healthcare Providers required.



Valid NJ Drivers License and clean driving record required.
